B. Does the facility have its own code of conduct? If so, please attach a copy.
C. Is the facility currently required to comply with other clients code of conduct?

D. Has the facility ever been audited for social compliance issues?
D.1 By whom? Date of last audit? What were the results? Please attach a copy of the last audit report and/or summary of findings?

1. Compliance with Local Laws 遵守當?shù)胤?br /> A. Does the facility have on hand, the latest version of the local labor regulations (labor code)?
B. What is the lowest wage that can legally be paid? State whether its daily, weekly or monthly.
B.1 What is the minimum wage applicable to workers in this facility?
B.2 Are there any workers making less then MW? If yes why?
B.3 What percentage of workers are receiving MW?
2. Child Labor 童工
A. Does the facility employ workers under the age of 18?
A.1 If yes, how many? What are their ages? What types of work do they do?
B. How does the facility verify the age of workers?
3. Harassment and abuse 騷擾和虐待
A. Are managers and supervisors given skill training on how to supervise workers and compliance issues? If so, when was the last training?
B. Can workers file complaints about their supervisors and co-workers anonymously or confidential manner?
C. Are workers and supervisors made aware of sexual harassment policies and complaint procedures? If so, how?
4. Health and safety PVH驗廠健康與安全要求
A. Is there a health and safety manager in the facility? What is his or her name?
B. Is there a health and safety committee in the facility? And who are they? If there is, please attach a copy of minutes of last meeting.
C. Does the facility have at least two forms of egress (fire exits) remote from each other, on each floor?
D. Are all emergency exits clearly marked, unlocked and unobstructed?
E. Does the facility have an emergency evacuation plan?
F. Please attach a copy of the emergency evacuation plan.
G. Does the facility have proper lighting, ventilation and temperature control?
H. Do workers have free access to clean potable water at all times?
I. Do workers have unrestricted use of bathroom facilities that are clean, stocked and sufficient in numbers?
5. Nondiscrimination PVH驗廠對歧視要求
A. Does the facility prohibit or in anyway restrict the employment of workers based on age, race, gender, sexual orientation, national origin, religion, political affiliation, membership in a union or otherwise?
B. Does the facility make all decisions regarding hiring, promotions, job assignments and terminations based solely on a persons qualifications?
6. Wages and benefits pvh驗廠對工資和福利的審查
A. Does the facility maintain payrolls where all the people working in the facility are listed?
A.1 Do workers get a pay stub/receipt on pay day, which shows their total earnings, hours of work, benefits and deductions made? Attach a sample pay stub.
A.2 Do the payrolls indicate the total earnings of the worker, deductions from pay and number of hours worked each pay period?
B. List all benefits required to pay workers by law (i.e., vacation, holiday, health insurance, etc.) and indicate whether it is paid.
C. Does the facility give workers other benefits not required by law? Please list them.
7. Work Hours 工作時間
A. Does the facility maintain a time recording system which is used by all production workers? List type: (time clock, swipe in electronic system, etc.)
B. Does the facility limit the number of weekly work hours to no more then 60 in one week and no more the 12 in one day?
C. List the highest number of hours worked, in one week, for the last year. List by worker's name, hours worked and date.
D. Are workers allowed at least one day off for rest every single week? Please attach a recent sample copy of hours work records
8. Overtime 加班
A. Are overtime hours worked voluntarily?
B. Are workers paid overtime in accordance with legally required premiums?
C. Are overtime hours recorded on the payrolls?
C.1 What was the highest number of hours worked in one week during the last year?
9. Freedom of Association 結社自由
A. Is there a union in this facility?
A.1 Do they have a collective bargaining agreement?
B. Are workers free to join unions and or groups that advocate better working conditions and worker rights?
10. Forced Labor 強迫勞動
A. Does the facility employ prison labor or bonded labor?
B. Are workers free to go in and out of the factory with little or no restrictions?
C. Are emergency doors unlocked from the inside at all times while workers are in the facility?
11. Environmental Conditions pvh驗廠對工廠環(huán)境條件的要求
A. Is waste properly disposed of, and not simply dumped into the environment?
B. Water waste from dyes, tints, chemical or solvent usage is treated before disposing?
C. Chemical and solvent containers are properly disposed of?

A. Does the facility use subcontractors?
B. What operations are subcontracted?
C. Please submit the names and addresses of all subcontractors
13. Homeworkers
A. Is any part of the production done inside a worker's home?
B. How many home workers does the facility employ in a typical week?
C. How are homeworkers compensated?
IV、 Right of PVH to conduct compliance audits:
A. Does the facility agree to abide by the code of conduct of PVH?
B. Does the facility agree to allow PVH's internal and external compliance auditors to monitor facility's labor conditions on a regular basis?
C. Does the facility agree to allow announced and when needed unannounced compliance audits and to immediately remediate non conformance issues?
D. Does the facility agree to cooperate 100% with reasonable auditors' request for company records, workers to interview and full access to all parts of the facility?
